Buttock or Gluteal Anatomy

There are 3 main muscles that make the gluteal muscle region. The gluteus maximus muscle which is the largest of the three. It is also the most superficial or towards the surface. It produces the shape of the buttocks. Its function is to extend the thigh. It attaches to the ileum, sacrum and coccyx (pelvic area) and extends downwards at a 45 degree angle and inserts into the tuberosity of the femur (hip area). The gluteus medius muscle and gluteus minimus muscle are the other two gluteal muscles and smaller. They do not alter the shape of the buttock.

Butt Augmentation Surgery

Buttock enhancement can be achieved by two ways. The more common fat transfer or Brazilian butt lift and solid silicone implant placement (butt implants).

Fat based or grafting: The brazilian butt lift BBL augmentation is performed by harvesting fat by liposuction from a different area of the body and transferring it to the buttock area. Fat survival can be an issue and more than one fat transfer may be needed to obtain the optimal look. In this procedure fat is transferred from another area of the body to the buttocks by injection techniques. Liposuction around the buttock area is also performed to sculpt the surrounding area and therefore enhance the overall shape. Fat is removed by liposuction and then injected to each buttock enhancing the size the shape. The buttocks can be shaped to be more round and the projection can be increased. This procedure can make your body more balanced with the rest of your figure. If you do not have enough fat for harvesting and transferring a buttock implant may be an option for you.

Implant based: Solid silicone butt implants are placed in between the gluteus maximus muscle. The pocket is created and the implants are placed in position. No drains are used.

Incisions are placed in the gluteal cleft on either side. The silicone implants generally come in 3 sizes small, medium and large. They can be round or shaped. The implants can also be carved to customize the shape if needed.

It is important to see a surgeon with knowledge of the anatomy of the area and fat grating or transfer techniques. All of this can affect your results and safety. Candidates for Butt Augmentation

Ideal candidates are healthy. Candidates generally want to improve the shape of their buttock area or want more size and projection. Fat transfer candidates must have an adequate donor site fat to harvest. Liposuction is combined with buttock enhanced to improve the silhouette and contour of the overall hip, lower back, and thigh region. Sometimes more than one fat transfer is necessary to reach a desired goal. Patients that do not have enough fat may choose to undergo gluteal or butt implants. These options will be discussed in detail with DR. Parikh at the time of consultation and is important to ensure high patient satisfaction.

Sculptra Butt Lift

Buttock Enhancement or Augmentation compromises a variety of techniques. The buttocks and hips can be accentuated with increasing the volume in these areas as well as liposuction or fat removal in other areas. In the right hands, the combination of techniques can significantly alter the shape and physique of the waist, hips and buttocks with the common goal of obtaining the curves and contour you desire.

Restoration of volume of the butt or hips can be performed several ways. The two most common ways and recognized standards in the United States are;

  • Fat Transfer to the Buttocks of Brazilain Butt Lift aka BBL
  • Buttock or Gluteal Implants

The above two mention techniques have consistent results and are recognized as acceptable methods to enhance the buttock by the plastic surgery community at large. Dr. Parikh performs both Fat Transfer and Buttock Implants routinely.

Sculptra on the other hand is a filler material that is injected into the buttock or hip area in effort to gain volume in these areas. In general it is a long acting filler and historically described for the use in facial rejuvenation. This has expanded into the area of buttocks.

Sculptra How it Works

Sculptra or Scupltra Aesthetic is made of injectable poly-L-lactic acid. This is a filler product that is long acting and in general, can last over 2 years. There are reports that in the buttocks the filler can last between 5 – 7 years. Sculptra once injected into the area acts as a collagen stimulator. Therefore it takes time to show results. Over time more collagen is built and therefore helping to restore the volume.

Sculptra How is it Performed

Dr. Parikh Board Certified Plastic Surgeon performs the procedure as an outpatient in the office and takes about 15-20 minutes. The procedure is performed with local anesthetic or numbing shots, followed by injection of Sculptra to the areas. The procedure can also be performed with sedation or general anesthetic if one chooses.

Preparing for Buttock Sculptra

Your goals should be set, a thorough consultation regarding what areas you want to improve and what areas bother you the most is important. Multiple fillers may be needed to reach your desired goals and this will be discussed during your consultation. Also medications or over the counter supplements that thin the blood can increase the risk of bleeding and bruising. These medications should be stopped a minimum of a 1 week prior to filler treatment.

Post Procedure Recovery

Results can be seen immediately after injection. However this is a temporary effect. The results will slowly disappear at first as the swelling and fluid absorbs. Within weeks to months noticeable results should be seen. Swelling and bruising can occur sometimes and be seen soon after the procedure. This should reside within a week. There are no restrictions and it is a quick office based procedure.

How much is Needed

Commonly 10 vials are used per session. If larger results are desired, a second treatment is required about 8 weeks apart. A candidate must weigh the cost of the procedure and the possibility of future procedures as needed.

    A simple way to go about this is to see if you are a candidate for fat transfer first. Ideal candidates for fat transfer or BBL have adequate amount of donor sites of fat to harvest from. Meaning, the patient has enough fat to take from other parts of the body to transfer which will achieve the desired result of the patient. If the candidate does not have enough fat to transfer then butt implants are a great option. Butt implants do not change volume over time. The volume put in will stay that way.   • What area the different techniques that Butt Implants are Performed?

    The placement of butt implants can be at 4 planes;

    • Below the Gluteus Maximus Muscle

    • Above the Gluteus Maximus Muscle and Below the Fascia
    • Above the Gluteal Fascia and Below the Skin

    Butt Augmentation Surgeon Dr. Parikh prefers to place the implants within the gluteus maximus muscle. This allows for more soft tissue coverage over the implant. Therefore hiding the implant better and giving a more natural appearance. The other techniques are generally not performed by Plastic Surgeon Dr. Parikh but may be techniques elsewhere.

  • Can Butt Implants and Fat Transfer be performed at the same time?

    Yes, implants and fat transfer can be performed at the same time if needed for shaping and additional volume. Frequently liposuction of the lower back and hips is performed simultaneously with implants to improve the overall aesthetics. Fat transfer can be used to place on the hip area to round the sides of the buttocks where frequently it is squared off. Fat transfer can also be placed over the implants to add projection. The implant is a solid silicone and rupture of the implant is not usual compared to a breast implant which is composed of gel silicone.

  • Are Butt Gluteal Implants Safe?

    Yes Gluteal Implants are safe. They are composed of a solid silicone that can be carved and cut without leakage of silicone. In the right hands complication rates can be minimized and can lead to a high overall satisfaction rate and happy patient.

  • Are Butt Implants easy to Remove?

    Yes, the implants are easy to remove. They can be removed through the old incisions. There is a low downtime and recovery after the procedure.

  • Where are the incisions located for Butt Implants?

    Typical Gluteal Implant Scars or Incisions include;

    • One Incision in the Gluteal Cleft
    • Two Incisions in the Gluteal Cleft

  • What is the downtime or recovery time after Butt Implant Surgery?

    The downtime for butt implant surgery is 2 weeks of strict no sitting on the area at all. This seems quite difficult to do but it is important in the obtaining the best possible outcome with your new butt implants. After time your body will adjust and the swelling will come down. You may not even notice they are there.

  • What are the Complications of Butt Implants?

    The complications of butt implants include;

    • Infection
    • Wound Separation
    • Malposition
    • Hematoma
    • Pain

    These complications can not common but can occur. Butt Implants are easily removed if required.

  • What are Butt Gluteal Implants made out of?

    Solid Silicone material. They feel harder than a breast implant but softer than a pectoral implant. They feel slightly form and come in different sizes and shapes.

  • Can I feel the Butt Implants after the Surgery?

    If there is a good amount of soft tissue coverage over the implant and the implant is is good position it is difficult to feel them. Putting the implants within the muscle increases the soft tissue coverage over the implant and therefore masks the implants edges and ability to feel.

  • What does Brazilian Butt Lift aka BBL mean?

    A Brazilian Butt Lift consists of removing fat from areas of the body by liposuction and injecting it into the buttock area to enhance the shape and size of the buttocks. This fat can be harvested from tummy, hips, thighs, back or arms. Once the fat is harvested by liposuction it is processed and injected by cannula into the buttocks and hip area if needed for contouring.   • Can BBL be combined with other Procedures such as Tummy Tuck?

    A BBL procedure can be combined with buttock implants if needed. Liposuction is part of the BBL procedure for harvesting fat. Additional areas can undergo liposuction if needed for further body contouring. Abdominoplasty and BBL at the same time is a common scenario. The two can be done together however the recovery will be more intense as well as the operative time.
